FSD5 would like to thank you for your interest to volunteer and/or chaperone.  

  

· Volunteer Level I: To be approved for this level, the volunteer participant must be in the presence of and supervised by FSD5 employee at all times. The volunteer must have a National Sex Offender Registry check before (s)he volunteers at the school.  

 

· Volunteer Level II: To be approved for this level, the volunteer chaperone understands that this status allows interaction with students without a FSD5 employee immediately present.  The volunteer applicant must have a National Sex Offender Registry check before (s)he volunteers at the school, in addition to a criminal background check. This volunteer must also complete the safety video trainings (link provided below).

 

· Approved volunteers should familiarize themselves with the district polices/rules, school handbook, and school routines and procedures prior to volunteering at any of FSD5 schools. Volunteers who fail to follow district/school policies and procedures or jeopardize the safety of a school or students may lose volunteer status, be denied access to the school, or be subject to other consequences. The instruction of and discipline of students is the responsibility of the teacher and cannot be delegated to the volunteer.

 

· Volunteers must sign-in and sign-out and are required to wear and clearly display the school-issued temporary volunteer tag.

 

· If a volunteer is injured while on school property or providing volunteer services, (s)he should report the injury to a school administrator.
